The fee is $5 and you will need your Vehicle Identification Number, driver's license number and other information. This transaction is for individuals only. Businesses and Trusts listed as the registered owner must order by mail, by fax or in-person.

A duplicate Certificate of Registration or plate decal can be obtained for a fee of $5.

You must have a copy of your Nevada Evidence of Insurance Card. You may mail the application, card and a check, money order or a Payment Authorization (VP-205) to the address below. You may also fax the form, card and an authorization to (775) 684-4797.

If you need to change your address, you must also include complete a Change of Address Notification by Mail (DMV 022). See Address Changes. A postal forwarding order does not change your address with the DMV.
