DMV driver history records are separate from court and criminal history records. Changes and time limits in your driver history printout do not affect any records kept by courts or law enforcement.

Pictured below is a sample 10 year history obtained from the DMV Records Section. Driver histories obtained from the DMV web site are 3-year histories and do not include withdrawal actions. Click on the graphic for information about an item.

Here is an explanation of the actions which would have occurred, in chronological order, to generate the listings in the sample record of Sam A. Driver III shown above.
Sam failed to appear at a court hearing in 1999 and the court issued a bench warrant for his arrest. This resulted in Withdrawal Listing 1 (FTA - Failure to Appear). Sam's license would have been suspended on 12-02-1999. However, he cleared the warrant on 10-16-1999 and no action against his license was taken. There was no conviction of any traffic offense in this case.
On 03-15-2001, Sam was cited by law enforcement for Driving with No Insurance. He was convicted of this offense on 04-24-2001. This resulted in Conviction Listing 1 (violation code 019 - Driving with No Insurance). Nevada law specifies a driver license suspension for this offense. Hence, the conviction resulted in Withdrawal Listing 2 (NIA - Driving with No Insurance). Sam's license was suspended on 06-15-2001.
As required by law, Sam surrendered his driver license and received an Identification Card. There is no indication of this on the printout, however, due to the fact that Sam had subsequent suspensions and was issued an ID card at a later date.
On 7-20-2001, Sam met the reinstatement requirements to regain his driving privilege. He surrendered his ID card and received a driver license. This resulted in the entry under "NCDL Reinst Dt" in Withdrawal Listing 2. Sam's license was suspended from 06-15-2001 to 07-20-2001.
Nevada law requires motorists to file SR-22 Proof of Financial Responsibility insurance coverage as part of the reinstatement requirements for a no insurance offense. In early 2002, Sam's insurance company notified DMV that he had failed to maintain this coverage. This resulted in Withdrawal Listing 3 (FMC - Failure to Maintain Coverage). Sam's license was suspended on 03-18-2002. Sam surrendered his driver license to the DMV and received an Identification Card on 03-19-2002.
Sam failed to appear for another court hearing in 2002. The court issued an arrest warrant and the DMV issued another license suspension on 06-11-2002 (Withdrawal Listing 4 - FTA). Motorists can have any number of license withdrawals in effect at the same time. They must meet the reinstatement requirements for all of them to regain driving privileges.
On 09-01-2002, Sam met the reinstatement requirements for both the Failure to Maintain Coverage and the Failure to Appear suspensions. He surrendered his ID card and received a driver license. This resulted in the Reinstatement Date entries in Withdrawal Listings 3 and 4. Sam's license was suspended from 03-18-2002 to 09-01-2002.
On 09-15-2002, Sam was cited by law enforcement in Oregon for speeding. He was convicted of this offense on 10-15-2002, resulting in Conviction Listing 2 (Violation Code 431 - Speeding 21 MPH or More Over the Posted Limit). This conviction resulted in 4 demerit points. Nevada law does not require a license suspension for this offense and no withdrawal action was taken.
On 09-30-2002, Sam met the requirements for a Class M Motorcycle license and received a new license with both Class C and Class M listed.
In late 2002, Sam's insurance company again notified the DMV that he failed to maintain his SR-22 insurance coverage. His license will be suspended on 01-20-2003 as shown in Withdrawal Listing 5. The date of this sample record is 01-02-2003. Sam's license remains valid on this date.

two years. The driver must obtain a new exam by the date listed. This
will show all zeros if the driver has never filed a Commercial Driver License
physical.

Permit - Indicates whether the license is a
normal driver license, an instruction permit or an ID card.

Issue Date - The latest date a valid license or
card was issued. May be the date of original issue, a renewal, a
duplicate, an address change or another action such as adding an
endorsement.

Expiration Date - The date the license will expire. Licenses expire on your birthday four years after issuance or, if applicable, on the date of immigration expiration/departure. See Driver License

Restrictions and Endorsements - Restrictions
placed upon driving privileges or endorsements granting the driver
extra privileges. Restriction A requires the driver to wear glasses or
contact lenses. Endorsement F is a limited commercial licensing
exemption which allows firefighters, farm employees or active duty
military members to drive large vehicles related to their duties. See Nevada Administrative Code Chapter 483 for a complete
list of restrictions.

Stop Code - Indicates the withdrawal action was
stopped before the license was actually suspended, revoked or
cancelled.

Court Code and Case/Citation Number - The court
which has jurisdiction over the traffic citation or criminal
conviction and the case number on file with that court. Only
convictions are reported to DMV. Cases which are dismissed or reduced
to non-moving violations are not reported. Out-of-state courts are listed as 999.

Begin Date - The date a license withdrawal
began or was scheduled to begin.

End Date - The date that a license withdrawal was cleared or the date that a withdrawal was scheduled to end if the offense has a specific period outlined in Nevada law. For example, a first DUI conviction carries a specific suspension period of 90 days. Other offenses, such as Failure to Appear, have no set periods. Note: The End Date is not an indication that a motorist's driving privileges have been restored. The driver must follow all reinstatement requirements and physically receive a license. The reinstatement will then be listed under "Reinst Dt".
If the end date listed is prior to the begin date, the
motorist cleared the withdrawal action before the license was actually
suspended or revoked. No license withdrawal occurred.

Status - Indicates the
type of license withdrawal, i.e. Suspended, Revoked or
Cancelled. "Cleared" indicates that a potential withdrawal
was cleared before any action was taken.

Reason Code - These are the codes reported to
the national Problem Driver Pointer System and available to all 50
states. They are essentially the same as the Nevada
Withdrawal Codes. BLK stands for blocked, meaning no
withdrawal action was taken.

Ext - Extent - Whether the
withdrawal affects both Non-Commercial and Commercial licenses. Some
violations which cause a withdrawal on a commercial license do not
affect the motorist's privilege to drive non-commercial vehicles.

State - Shows the state
which initiated the withdrawal.

Reinst Dt - The date the
motorist reinstated his Nevada driving privileges and physically
received a driver license. NCDL = Non-Commercial
License. CDL = Commercial License.

Conviction Listings - These are listings
of moving violations in which the motorist was convicted of the
offense by a court of law. DMV is not notified of offenses which are
dismissed or reduced to non-moving violations.

Demerit Points - The
number of active demerit points. Points are assigned upon conviction
of an offense and expire 12 months after the conviction date. An accumulation of 12 or more points within 12 months
results in a license suspension. The first number in the violation
code (Viol Code) indicates the number of points for that offense. See Demerit
Points for more information and Code Search for listings of offenses and points assigned.

The dates the citation (Cite) was issued by law enforcement and the
date the motorist was convicted (Conv) by the court of jurisdiction.

and Haz Mat - Indicates whether the offense was committed in a
commercial motor vehicle (CMV) and whether the vehicle was carrying
hazardous materials (Haz Mat).

Citation Number - The
case number on file with law enforcement and the court. If the offense
resulted in a license withdrawal, the case number here should match the
number in the Withdrawal Listings.
