Advanced Secure Issuance (ASI)
The DMV has implemented its new ASI licensing program at all offices except the Fallon office, which will begin Thursday March 18.

Start on our Proof of Identity page if you are applying for a first-time license or identification card or for other major changes such as a name change.

ASI does not affect renewals, duplicates or address changes. See our ASI Home Page for more information on this program.

There is a $5 fee for each certificate requested. You may mail this application and a check, money order or a Payment Authorization to the address on the form. Satisfies smog check requirement for registration renewals.

Certifies that a specific vehicle has not been operated on Nevada public highways.

For Vehicle Identification Number Verification. Must be completed by a DMV representative, a peace officer or other authorized agent. (Not Fill-In)
Registration Fee Refund Request Form (VP-210)
To request a refund under certain circumstances

Designation of Use for Veterans Exemption (VP-187)
Used to designate that a portion of a Veterans Exemption be directed to the Nevada Veterans Home Account.

Trust Affidavit for Exemptions (VP-242)
To enable the use of a Veterans or other exemption when the vehicle is titled to a trust.

Non-Resident, Active Duty Military Governmental Services Tax Exemption Affidavit (VP-203)
For active duty military members who are residents of another state and stationed in Nevada.

Restored Vehicle Odometer Certification for Emission Exemption (EC-18)
To claim the Emissions Inspection Exemption for vehicles registered as a Classic Rod or Classic Vehicle.

Dormant Vehicle Affidavit (NVL-003)
Used to reinstate a vehicle registration which has been suspended for failure to maintain liability insurance. Only used if the vehicle has not been operated during the lapse of insurance.

Application for Individual Record Information (IR-002)
Anyone who requests driver license or registration records must complete this packet.  Motorists who need copies of their own records should visit a DMV office or contact the Records Section as listed in Driver History.

Application for Records Account (IR-001)
For insurance companies and other businesses which request individual DMV records on a regular basis.

Letter of Authorization to Release Information (IR-015)
An affidavit that allows customers to grant third party access to their records.

Please contact the Records Section for fee quotes and other details if you desire statistical reports or a large number of individual records. The release of DMV records is governed by Nevada Revised Statute 481.063 and the National Driver Privacy Protection Act.
