Advanced Secure Issuance (ASI)
The DMV is implementing its new ASI licensing program from January through March, 2010. Documentation requirements will differ depending on whether the office you visit has implemented ASI.

Driver
License Application (DLD-1)
For an original Nevada driver license at non-ASI offices. Must be submitted in person.
Do Not Mail This Form! Mailed applications are rejected.
Minor
Affidavit (DLD-38)
Certifies that a minor is aware of various requirements and laws.
Also used by a parent/guardian to cancel a minor driver license or instruction
permit.
For Commercial Driver License US DOT exam requirements.
Current license holders who are filing
only an updated physical may mail the form to the
nearest CDL
Office or deliver it in-person at any CDL or Full
Service Office.

Certifies that a specific vehicle has not been operated on Nevada public highways.
Vehicle
Inspection Certificate (VP-15) For Vehicle Identification Number Verification. Must be completed by a DMV representative, a peace officer or other
authorized agent. (Not Fill-In)
Dormant Vehicle
Affidavit (NVL-003)
Used to reinstate a vehicle registration which has been suspended for failure to maintain
liability insurance. Only used if the vehicle has not been operated during the lapse of insurance.
This form is for use only if the vehicle was last
titled in Nevada.
If you are not sure whether your vehicle is titled in Nevada or if you do
not have all of the information requested, please contact the Records Section for instructions.
If your vehicle is titled in another state, you must obtain a duplicate
from that state.
You must file this form within 10 days of any accident which was: (1) not investigated at
the scene by law enforcement and (2) caused injury, death or more than $750 damage.
